Medical Consent

I certify that: (1) I am not currently taking nitrate medications in any form; (2) I have accurately disclosed all current medications and health conditions; (3) I understand that compounded PDE5 inhibitor formulations are not individually FDA-approved but are prepared in FDA-regulated compounding pharmacies; (4) I understand the potential side effects including headache, flushing, nasal congestion, and in rare cases vision or hearing changes; (5) I will contact a physician immediately if I experience chest pain, sudden vision loss, or a prolonged erection (priapism lasting more than 4 hours).
