Medical Consent

I certify that: (1) I have disclosed all relevant medical history to the best of my knowledge; (2) I understand that bio-identical hormone therapy carries potential risks including (but not limited to) breast tenderness, spotting, fluid retention, and in rare cases more serious cardiovascular or clotting risks, which will be reviewed with my physician; (3) I will notify my physician of any changes to my health or new diagnoses; (4) compounded BHRT formulations are not individually FDA-approved but are prepared in FDA-regulated facilities; (5) I will maintain regular follow-up care including periodic lab monitoring as recommended.
